| The importance of having an Advance Directive |
| An Advance Directive varies from state to state. It is a written directive for making health care decisions. In most states this directive is two-part. One being your "Health Care Instructions" and another called "Appointment of Healthcare Representative." You should make a list of those who have a copy of your Advance Directive. This might include your family members, health care representative, physician, attorney, spiritual advisor, hospital or other health care facility. |
